The information you provide to WCPS via the ‘Contact Us” page or the ‘On-line Referral Forms’ will be used solely to communicate with you. Your contact information will not be added to any internal mailing list, nor will it be sold or shared with any other party.
Although ever effort is made to ensure on-line security, WCPS cannot guarantee the security of any information sent to WCPS using e-mail. Confidential or sensitive information should not be sent via e-mail or by any on-line mode of communication. WCPS will not be liable for any breach of confidentiality resulting from your communicating to WCPS through this website. After receiving your contact information, WCPS will connect with you to set up an in-person or telephone meeting where you can share confidential or sensitive information.
In case of a privacy breach to the website, Dr. Joseph Klancnik, WCPS Director would notify the WCPS IT department immediately after the breach was discovered. Steps would be taken to minimize damage. These steps may include changing passwords, changing identification numbers and/or shutting down the website.
